Connect Together - Children and Young People Social Prescribing (Closed Service)

Social prescribing service that supported children, young people and their families to make long lasting connections in their community to bring about sustainable change and improvements to their lives.

Who is the service for?

The Connect Together service supported children and young people, aged 5 to 16, to create a plan of what they wanted to be different, and supported them to achieve their goals. The team provided a tailored service, listening and responding to the young people they supported. Some interventions included:

  • providing emotional and wellbeing support
  • developing physical activities for children and facilitating the use of outdoor spaces
  • organising creative activities to fill community gaps
  • addressing weight management
  • helping with school readiness
  • supporting social and emotional skills
  • encouraging positive community engagement and fostering community links

With support from Connect Together, young people improved their:

  • Emotional and mental health (79%)
  • Ability to make good choices (70%)
  • Ability to manage problems and challenges (69%)
