Delivered in partnership by Barca-Leeds and Simon on the Streets, the Health and Wellbeing Navigators supported people to overcome the barriers they face in accessing health and wellbeing services, and work towards the person’s individual goals in relation to improving their health.
The service supported people who have experienced a lot of adversity in their lives, including homelessness, substance misuse, mental health issues and may have a history of offending behaviour and can find it difficult to engage in services. There is increased vulnerability to exploitation and harm which is particularly relevant for people who are rough sleeping, women, asylum seekers and refugees, and those who have no recourse to public funds. Through co-production, the Navigators listened to the people they supported to understand their health needs and consider how to support them to access the care they need.
The team supported people across Leeds and worked closely with Barca-Leeds Housing Navigators, Barca Outreach Support Team and Simon on the Streets Outreach Workers.
