Adult rough sleepers and young people (18 to 25) at risk of rough sleeping as adults.
Referrals are identified and allocated by the Street Support Partnership, chaired by Safer Leeds and Leeds Housing Options along with other delivery partners. This ensures that key agencies are working closely together to enable the choices of individuals that maximise successful outcomes and provide the right level of support.
Part of the Single Homelessness Accommodation Programme (SHAP), this new and innovative citywide, multi-agency housing support service works with adult rough sleepers and young people (18 to 25) at risk of rough sleeping as adults. The team work with partners to deliver a new and unique model of supported accommodation, that provides accessibility and choice for people with severe multiple disadvantage, disability and complex needs who would otherwise continue with a rough sleeping way of life.
A coproduction group of 30 people comprising: people we are currently supporting, volunteers and front line staff with ‘lived experience’ and Leeds City Council staff have co-created a Best Practice Guide for Commissioners in Leeds to establish a new model of provision.

SHAP is delivered across Leeds by a partnership of Leeds City Council, Bridge-It Housing, Turning Lives Around, St George's Crypt, Barca-Leeds, Leeds and York Partnership NHS Foundation Trust, Leeds Community Healthcare NHS Trust and Bevan Healthcare.
