Social Prescribing harnesses the power of people and supports the growth of community talents and assets by connecting individuals with a range of local community resources to improve social, emotional and mental wellbeing.
The Linking Leeds service believes that everyone can live a fulfilled life and our Wellbeing Coordinators can help everyone to explore opportunities and make positive changes. Feeling connected to your community has lots of health and wellbeing benefits that support people to realise their potential, independence and improve physical and mental health.
Wellbeing Coordinators are based within GP practices and local community settings providing one to one support over the phone and face to face. We take time to listen to ‘what matters to you’ to provide light touch or more holistic planned offers.
