Community Wellbeing Connectors

The Community Wellbeing Connectors enable people to increase their independence and support them to recover and improve their mental health and wellbeing.

Who is the service for?

The team supports individuals experiencing serious mental illness in West Leeds. Due to current capacity, the service is open to referrals only from the Transformation Hubs.

Referrals

Referrals are now accepted only via Care Director.

Priority Areas

Referrals are prioritised from:

  • West PCN (including Bramley area)
  • Armley PCN
  • Woodsley PCN

Other Areas

Referrals from other areas may be considered based on individual need and service capacity.

Please note: We do not accept self-referrals or direct referrals from GPs.

Referral Process and Capacity Updates

Before submitting referrals via Care Director, they should first be discussed with a community connector based within the Community Mental Health Team MDT meetings or by contacting the team at: westcwc.lypft1@nhs.net. Capacity updates will be shared during meetings as a worker is assigned to each area.

The team engage with individuals in their own community settings, build trusting relationships and provide one to one outreach care and support. Supporting each individual to identify and engage with activities, groups and resources in their local communities to build their personal resilience and a personalised local support network.

The Community Wellbeing Connectors are part of a service located in three initial sites as part of the Community Mental Health Transformation for Leeds. This is a partnership of NHS organisations, Voluntary and Community Sector agencies and Leeds City Council Adult Social Care. The service work closely with people who have lived experience to transform how primary and community mental health services for people experiencing severe mental health issues are organised and delivered.

"The Worker helped me to get back into the gym, she supported me through things that I couldn't cope with like helping me sort my bills out and supporting me through my sons passing which she didn't have to do, but she listened to me then passed me onto the bereavement counselling which helped me. I never had that much support and her visits will be missed so thank you for everything most appreciated."
